The query said mobile and the results did not

This is the part that matters more than the order.

A patient typing “mobile botox” is expressing a specific preference: they want the appointment to come to them. Google answered with three clinics they would have to drive to. Either no mobile provider in that market has a profile positioned to answer the query, or the ones that exist are not configured in a way that surfaces for it.

Both explanations point the same direction for a provider who does travel. If the service is genuinely mobile, the profile has to say so in the places Google reads — the service area, the category, the services list, the description — rather than only on a website.
